sole proprietorship to single member LLC?

Thank you very much for your help.

Should I convert my sole proprietorship (and EIN #) to a single member LLC? if so, how do I estimate the "fair value market"?
Or should I form a new single member LLC?
I have no debts, I offer services, I do have assets I would like to transfer to the LLC.
