Now that
you have a form SS4, it's time to complete it and file it with the IRS.
Let's
begin with the first item:
This one is
fairly self-explanatory: just fill in the name of your LLC (with the
appropriate ending that you used when filing your articles of
organization).

This
next question
is asking for the trade name of the business. If you have not filed a
DBA for your LLC to do business under a name other than the name of
your LLC, then leave this blank. If you don't understand what the last
sentence just said, then leave this question blank.

This
is just
asking for the name, address, and so on of your business. If you have
hired a registered agent, you can enter the registered agent's
information here. Otherwise, just use the location of your business.
This can be your home address if you operate your business from home.

Enter
the name of
managing member of your LLC. In part 7b, enter the social security
number of the managing member. If the LLC is owned by a trust or by
another LLC, then enter the trust identification number or EIN of the
LLC.

Notice
that there
is no box for an LLC. Just check "Other" on the bottom left and write
in "Limited Liability Co".

Enter
in the
reason for applying. You might have more than one reason, such as you
started a new business and you also want to open a bank account. Just
choose one reason that is true (never lie on
anything you submit to the government).

Part
10, enter the
date you started your business. In Part 11, you need to enter the
closing month of accounting year. Unless you chose differently, you
will probably use the calendar year. Therefore, enter December in box
11.
If
you have no employees, then leave Part 12
blank. If you have employees and have paid them, enter the date you
first paid them.
In
Part 13, estimate the number of
employees you expect to hire during the next 12 months. If you don't
intend on hiring anyone, enter -0- in each box.
Part
14 asked
you to enter your business type. If your business doesn't fit into any
of the categories, just check "Other" and fill in what your business
does.

In
Part 15, just
describe the product you sell or service you provide. There's not much
space, so just write something like "Web hosting services" or "lawn
maintenance" or whatever your business sells.

If
your LLC has
never applied for an EIN before, then you can check "No" for 16a and
leave the rest of 16 blank.

This
section is
asking if you want some other person to receive your LLC's EIN and
answer questions about the form, such as an attorney or accountant. If
you're doing this yourself, then leave this section blank.
